I hereby give my consent to being photographed and/or video recorded by the Institute of Infectious Disease and Molecular Medicine (IDM), Communications and Marketing Division, Faculty of Health Sciences, University of Cape Town (UCT). I understand that my views will be represented accurately and fairly. I understand that I have the right to withdraw my consent in writing at any time, and that any material bearing my image, voice, or identity will be removed from UCT resource libraries and will no longer be used in future marketing material, from the date my withdrawal is received. For this purpose, I grant the following rights:
